                                APPENDIX I
                           CALL FOR NOMINATIONS

The Elections Committee of Division 40 requests your input in the nomination of candidates for election to the following Division 40 offices-President-Elect and Member- at-Large. The nomination of candidates is one of the most important acts of Divisional citizenship, and you are urged to exercise your rights in nominating individuals to serve the Division. According to the 1997 Division 40 Bylaws Revision, “All persons receiving nominations for an office from 1% or more of the membership ... will be included on the ballot if qualified in accordance with these Bylaws and Division Policies and Procedures.” As of October, 1998, there are 4,064 voting members of Division 40. Therefore, each potential candidate must procure the nomination at least 41 voting members to be considered for the 1999 ballot.

Please give consideration to the nominations of Division 40 members to these positions (including self-nominations) and complete the ballot below. IN ACCORDANCE WITH DIVISIONAL POLICY (February, 1998), ALL NOMINATIONS MUST USE THIS FORM TO BE COUNTED. NO COPIED FORMS, FAX TRANSMISSIONS OR NOMINATIONS RECEIVED BY ELECTRONIC MAIL WILL BE CONSIDERED VALID.
