This series includes but may not be limited to photocopies of receiving reports received from Purchasing, photocopies of Internal Transactions sent to Accounting Services, LSU Business procedures received from Business Affairs, photocopies of transmittals sent to Office of Bursar Operations, computer-generated Operating Budget for Fiscal Year received from Budget and Planning, NCR copies of Purchase Orders sent to Purchasing, NCR copies of requisitions sent to purchasing, and computer-generated monthly ledger sheets received from Accounting Services.

RETENTION: Retain 3 years in office, then destroy by landfill.

 

APPOINTMENT BOOKS

This series contains annotated appointment books.

RETENTION: Retain 1 year in office, then destroy by landfill.

 

DOCTOR REFERENCE FILES

This series contains photocopies of annual reports received from Psychology Department, campus and oft-campus correspondence, photocopies of minutes of meetings, procedural guidelines, photocopies of budget reports, and photocopies of Operating Budget for Fiscal Year received from Budget and Planning.

RETENTION: Correspondence: Retain 5 years in office, then destroy by shredding then landfill. Budget records: Retain 3 years in office, then destroy by recycling. All other contents:

Retain in office while active or until superseded, then destroy by recycling.

 

GRANT PROPOSALS

This series contains grant proposals.

RETENTION: Retain in office 3 years after year of compilation, then destroy by shredding then recycling.

 

INDEX OF TEST LIBRARY

This series contains index of test library.

RETENTION: Retain in office while active, then destroy by

recycling.

 

INVOICES

This series contains invoices received from off campus.

RETENTION: Retain 2 years in office, then 5 years in Records

Center, then destroy by landfill.

 

MASTER TESTS

This series contains master copies of tests.

RETENTION: Retain in office while active, then destroy by

shredding then landfill.

 

OUTSIDE REFERRAL CARD FILE

This series contains cards with names, addresses and specialty fields of outside psychologists.

RETENTION: Retain in office while active, then destroy by landfill.

 

RECEIPTS

This series contains NCR copies of receipts.

RETENTION: Retain 2 years in office, then 5 years in Records

Center, then destroy by shredding then landfill.

 

RECEPTION AREA WORKING FILES

This series contains file lists, schedules, photocopies of timesheets sent to Department of Psychology, fee schedules, and service guides.

RETENTION: Retain in office while active or until superseded, then destroy by recycling.

 

STATISTICAL ANALYSIS REPORTS

This series contains computer-generated Statistical Analysis

reports received from System Network Computer Center.

RETENTION: Retain 3 years in office, then destroy by landfill.

 

STUDENT ASSISTANT FILES

This series contains photocopies of timesheets sent to Department of Psychology and schedules.

RETENTION: Retain in office while active, then destroy by recycling.

 

TEST CHECKOUT CARD FILE -This series contains cards with date, name of borrower, etc.

RETENTION: Retain 3 years in office, then destroy by landfill.

 

THESES AND DISSERTATIONS

This series contains theses and dissertations.

RETENTION: Retain permanently in office.

 

UNIVERSITY ADMINISTRATIVE SPECIALIST WORKING FILES

This series contains long distance telephone call log sheets, NCR copies of Requests for Duplication sent to Copy Services, copies of University Stores invoices received from University Stores, campus near-print informational material, photocopies of Independent Study forms sent to Department of Psychology, contracts with Independent Study students, file lists, and file notes.

RETENTION: Retain 3 years in office, then destroy by landfill.

 

WORD PROCESSING FILES

This series contains documents such as letters, messages, campus correspondence, reports, handbooks, and manuals recorded on electronic media such as hard disks or floppy diskettes a) when they are used to produce hard copy that is maintained in organized files or b) when they are maintained only in electronic form and they duplicate the information in and take the place of records that would otherwise be maintained in hard copy, provided that the hard copy has been authorized for destruction by the LSU Retention and Disposal Schedule.

RETENTION: a) When they are used to produce hard copy that is maintained in organized files: Delete when no longer needed to create a hard copy. b) When they are maintained only in electronic form and they duplicate the information in and take the place of records that would otherwise be maintained in hard copy, provided that the hard copy has been authorized for destruction by the LSU Retention and Disposal Schedule: Delete after the expiration of the retention period authorized for the hard copy by the LSU Retention and Disposal Schedule.

The following records were not included on the draft schedule because they were determined to be medical records. This puts them outside of the scope of University Archives records retention responsibilities:

 

CASE FILES

This series contains incoming telephone data sheets, background information sheets, Consent for Services forms, fee schedules, Consent to Release Information forms, Authorizations to Obtain Information forms, case contact records, progress notes, summary reports, file lists, therapy check lists, and supervisor reports.
