RETENTION: Retain 2 years in office, then 5 years in Records Center, then destroy by landfill.

This series includes but may not be limited to photocopies of campus correspondence, security reports received from Campus Police, code number lists, photocopies of invoices, photocopies of inventory forms, equipment location sheets, Work Requests sent to Facility Services, price lists, annual safety inspection reports received from Facility Services, catalogs, and requisitions.

RETENTION: Retain 3 years in office, then destroy by landfill.

 

STAFF LEAVE FILES

This series contains campus correspondence, computer-generated Personal Activity Reports received from Human Resource Management, leave slips, photocopies of 1-9 Employee Eligibility Verifications sent to Accounting Services, and photocopies of W4s sent to Accounting Services.

RETENTION: Retain in office 2 years after termination or until final disposition of charge or civil action, then destroy by shredding then landfill.

 

STAFF PERSONNEL FILES

This series includes but may not be limited to photocopies of completed appointment forms sent to Office of the Dean, photocopies of performance evaluations, photocopies of step-increases received from Human Resource Management, photocopies of leave

slips, photocopies of letters of recommendation, photocopies of

1-9 Employee Eligibility Verifications sent to Accounting Services, and photocopies of Position Descriptions sent to Human Resource Management.

STUDENT ASSISTANT PERSONNEL FILES

This series includes but may not be limited to photocopies of timesheets and photocopies of the following sent to Student Aid and Scholarships: Form 1-9 Employee Eligibility Verifications, W-4s, Loyalty Oaths, and Student Employment Processing forms.
